You will need to clear three Strongholds inDiablo 4’s Kehjistan region: Alcarnus, Altar of Ruin, and Omath’s Redoubt. Each of these Strongholds has its own challenges and bosses, but once you’ve cleared them all, you will gain access to an Altar of Lilith. Once you’ve cleared each Stronghold, you will gain access to a new public area and any vendors, side-quests, or Waypoints it contains.

Unlike dungeons, Strongholds can only be completed once. They’re intentionally leveled a little higher than the player, so be ready before heading into each one. Having abuild fit for The Gauntletwould help you out here, as dungeons and Strongholds hold similar challenges. No matter what build you run, get geared up and head to the first Stronghold in Kehjistan: Altar of Ruin.

Diablo 4’s upcoming Season 5 will see one of the franchise’s most iconic monsters reborn as a new flaming terror to challenge unsuspecting players.

All Strongholds In Diablo 4’s Kehjistan Region

Start With The Altar of Ruin To The North

You can reach this Stronghold by traveling northeast from the Iron Wolves Encampment. Once you arrive,you will need to destroy the pile of rocks towards the center of the Strongholdto enter the Altar of Ruin. Move to the northwest, then kill the Triune Worshippers to receive the first of three Ritual Chamber Keystones needed to open the Ritual Chamber.

The minimum recommended level for the Altar of Ruin Stronghold is 45.

Once you have all three, go to where you got the second keystone and place the stones in the door. This will unlock the Ritual Chamber, which houses Dark Cardinal Maldu, the boss of the Altar of Ruin. Defeat him; then, you will need tointeract with and rekindle the Wanderer’s Shrine in the southof the area to unlock the waypoint and clear the Stronghold. After finishing up here, you’re able to head to the next Stronhold; Alcarnus, to the east.

The Alcarnus Stronghold

South of Kehjistan’s Jirandai Waypoint,you will find the Alcarnus Stronghold. The Jirandi Waypoint is the easiest way to get there, but you can also take the long road from Zarbinzet, heading to the northwest along the road past Tarsarak. Once you arrive, you should explore and get a lay of the land before completing any objectives.

The minimum recommended level for Alcarnus is Lv. 50. You can do this under-leveled, but prepare for a challenge if you choose to do so.

The main objective for Alcarnus is tofree the town of the rotten influence of the necromancer Rashta, the Mad Surgeon. She seeks immortality from the plague, and Bloated Corpsefiends and Shambling Corpse monsters litter the path towards her Lair. Once you arrive in Alcarnus, head to the north.

On your way north, you will encounter blobs called Necrotic Masses; for right now, they’re immune to damage, so leave them be and keep making your way past the Healing Well into the round chamber. If yousurvived the brutal gameplay ofDiablo 3, you will remember this room as Maghda’s Lair of the Witch. Keep heading on and speak to Rashta, the Necromancer.

Once you speak with her, she will start to attack you.Fight until she becomes immune, which will happen at about 10% health.Her health bar will turn grey. After she’s done talking, she will disappear and leave behind an item called Medical Notes. Pick it up and listen to what it says; you will need to investigate her Lair to learn more.

In her chamber,there are three Research Notes you need to find.Once you’ve seen all three, you will need to head back into the main area of the Stronghold. You will encounter Fly Swarms, Pestilent Hosts, and Skittering Abominations alongside the enemies already here.

They’re creepy but not as spooky as thetrailer forDiablo 4’s new DLC: Vessel of Hatred. Defeat or avoid these enemies and interact with one of the Necrotic Masses from earlier.

You will need to slay the Simulacrum that appears after interacting with the Mass, then do the same for the two other masses in the area.Be careful; these Simlulacra are no joke to defeat.

There are three of them, and you can choose to run away from them to complete the other Masses, saving their fights until the end if you’d like.Either way, you will need to clear out all three.Once you’ve done so, head back into the Lair, where you first spoke to Rashta, looting enemies along the way.

Just outside the chamber,rekindle the Wanderer’s Shrine to complete Alcarnus.Now that this Stronghold is cleared, head to the final Kehjistan Stronghold to the southeast; Omath’s Redoubt.

Omath’s Redoubt Stronghold

Kehjistan’s Final Stronghold, Home to High Priestess Hadar

Unlike Alcarnus, Omath’s Redoubt is a pretty simple Stronghold.Investigate the bodies of the fallen that are scattered around Omath’s Redoubt,then travel to the center of the area. Here, you will need to see a memory of what happened to the dead, learning that High Priestess Hadar had them sacrifice themselves to keep Omath’s Redoubt standing.

After learning this, you need to defeat the High Priestess in battle. It’s a pretty straightforward fight, and once you’ve defeated her, you canrekindle the Wanderer’s Shrine near the front of the Strongholdto complete the quest.
